Your Role:

• Manage and oversee patient care plans

• Collaborate with healthcare teams

• Educate patients and families

• Perform assessments & adjust care plans

• Ensure accurate documentation

What You Need:

• Active RN license

• 1+ year of clinical nursing experience (home health preferred)

• Strong assessment & communication skills

• Ability to manage a caseload independently
